Crisis Management

Crisis Management services are designed to provide a rapid response to an acute decline in function. Available either in person or by telephone, clients will be connected with staff who will assist them in managing the current situation in the least restrictive and clinically appropriate way.

We understand the importance of providing a safe and supportive environment for the client and their family to navigate this difficult time. The goal is to stabilize the crisis and create or modify the care plan to determine the additional resources that may be needed.
